1. Once the Angel Bikes Donation Form appears in your Adobe Acrobat Reader you can select the "Print" icon which is in the upper right-hand corner of the screen.  Print your form, fill it out and submit it along with your other documentation.
2. To save the form: Click on the downward arrow icon in the upper right-hand corner, the form will be saved in the download area of your hard drive.
After you have downloaded or printed the Application form you can return to the Angel Bikes web site by clicking the back button on your browser.
For security reasons, all information regarding applicants is held in the strictest confidence. Application information is never placed in an Angel Bikes Inc. database that has Internet access. We do not sell, lease or give application detail information to any person(s) or entity who is not an authorized representative of Angel Bikes, Inc. For this reason we use a written application form which you can download, print and fill out. We will notify you via e-mail once we have received your application. It will be carefully reviewed. In the next step toward approval, you will be contacted by a local representative of Angel Bikes, Inc. who will further the qualification process, obtain more detailed information regarding your disability and confirm the type of Angel Bike you require. Our representative will work with you to develop a plan related to acquiring the Angel Bike, assembling it and making the presentation to you.
